• 3 copies of Passport size Photograph of applicant
• Proof of Income: Salary Certificate/Pay Slip signed by Manager Accounts/HR/Admin/Finance/Personal or equivalent (for salaried person).
• Income Assessment/Declaration on official letterhead (for Businessman/Self-Employed persons).
• Other income documents/Papers(if any)
• Photocopy of passport (for International Card).

• Photocopy of latest Income Tax Return/TIN certificate
• Bank Account Statement: Last 6 (six) months.
• Photocopy of Passport/Electricity bill/WASA Bill/Telephone bill/Driving License/Registration/Fitness Certificate of own Car.
• Income from Rent: Copy of Valid Rent Agreement(if any)
• Card Statement: Copy of other Bank’s card statement for last 3(three) months (if any)
